Three edges of blood
Fit the same physical-damage weapon with three gems, each with a positive physical ATK +% primary effect.
All three primary effects must be percentage bonuses to physical attack. Values are unrestricted; no gem may convert the weapon to elemental damage. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
The fortified hammer

The Church Pick has thrust attacks. Each gem must boost this damage type as its primary effect. The three distinct gems must be fitted simultaneously to the same weapon on the same character.

The blazing cleaver
Fit a Saw Cleaver with three fire gems and confirm that its details show a positive fire attack value.
Each gem must have fire as its primary effect, either as a percentage or as a flat bonus. The Saw Cleaver has no innate element and must show a positive fire attack value once the gems are fitted. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
The lightning saw
Fit a Saw Spear with three bolt gems and confirm that its details show a positive bolt attack value.

The arcane cane
Fit a Threaded Cane with three arcane attack gems and confirm that its details show a positive arcane attack value.
Each gem must have arcane attack as its primary effect, either as a percentage or as a flat bonus. No fire or bolt effect may alter this conversion. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
The axe of the depths
Fit a Hunter Axe with one Heavy Abyssal gem that improves Strength scaling and two gems with physical ATK +% as their primary effect.
The Heavy Abyssal improves Strength scaling; the other two gems increase physical ATK by a percentage. The Headless Bloodletting Beast can drop the first gem at the bottom of depth 5 Chalice Dungeons. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
From cold blood to flame
Fit a convertible physical weapon with two Cold Abyssal gems that improve Arcane scaling and one gem whose primary effect increases fire attack. Confirm that the completed weapon shows a positive fire attack value.
The two Cold Abyssal gems improve Arcane scaling. The third gem provides the fire damage. The two blades of Cainhurst
Fit a Lost Chikage with one circular blood ATK +% gem and two radial physical ATK +% gems.
The Lost Chikage has two radial slots and one circular slot. Physical gems also boost its blood damage; the bloodtinge gem completes this setup for its transformed mode. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
The blood-fed Chikage
Fit a Chikage with three gems whose primary effect is blood ATK +%.
Bloodtinge droplet gems fit all of the Chikage's slots: the Mensis gem can be collected across several cycles. Out-of-shape Chalice Dungeon drops provide another option for non-circular slots. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
Three stones of slow poison
Fit the same trick weapon with three gems, each providing a positive slow poison buildup bonus.
Each gem may have slow poison as a primary or secondary effect. You do not need to poison an enemy or reach a specific buildup threshold. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
Three stones of rapid poison
Fit the same trick weapon with three gems, each providing a positive rapid poison buildup bonus.
Each gem may have rapid poison as a primary or secondary effect. Slow poison does not replace this effect; no threshold or fight is required. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
Four pulses of blood
Fit three HP regeneration gems to the trick weapon and a fourth to a left-hand weapon with a gem slot, with both weapons held simultaneously.
You need four distinct gems with HP regeneration as a primary or secondary effect: three in the right-hand weapon and one in the left-hand weapon. None may carry an HP-draining curse. Both weapons must be held at the same time.
Three breaths preserved

The Powder Kegs' charge
Fit a Stake Driver with three gems that each boost charged attacks.
The Stake Driver embodies the Powder Kegs' brutal ingenuity. The three charged-attack bonuses may be primary or secondary effects; no damage threshold is required. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
Three drops in steel
Fit three droplet gems simultaneously into the three slots of the same trick weapon.
Droplet gems fit every slot shape. Their effects are unrestricted: this achievement requires three gems of this shape. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
The three imprints
Fit a weapon with radial, triangular and waning slots with one gem of each of these three shapes.
The Lost Holy Moonlight Sword, for example, offers one slot of each required shape. Droplet gems do not count for this setup. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
Three fragments of the abyss
Fit the same trick weapon with three rating 20 abyssal gems compatible with its slots.
Abyssal gems have a rating of 20. Their effects may differ, as long as all three gems fit the weapon's slots. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
Three stones, one curse
Fit the same trick weapon with three cursed gems that all carry the same curse family.
All three curses must belong to the same family, such as increased stamina cost. Their numerical values may differ. The three distinct gems must be fitted simultaneously to the same weapon on the same character.
The 27.2% edge
Fit the same trick weapon with three cursed gems, each with physical ATK +27.2% as its primary effect.

Strength in stone
Obtain a Blood Gem on this character with a flat physical ATK bonus (Odd Tempering) as its beneficial primary effect.

Arcane radiance

This health recovery is passive. The Tear Blood Gem qualifies; rally, which restores health by attacking, is a separate effect. Shape, rating and strength are unrestricted; a secondary effect alone does not count.
Reclaim your blood
Obtain a Blood Gem on this character with increased rally potential (Lethal) as its beneficial primary effect.
The triangular gem in the Research Hall, among the bodies after rotating the staircase, has Lethal as its primary effect. Other documented examples only have it as a secondary effect. Identifying the effect is enough; you do not need to calculate the actual rally amount. Shape, rating and strength are unrestricted; a secondary effect alone does not count.
